Employee Complaint Form Template

Allow employee to write descriptive answers using text based questions. They can type the whole incident. They feel heard and that builds trust with the HR.

The form gives a breathing space between the answers by questions spearing one at a time. You can also add a text message before every question either as a greeting or to guide them to the next.

There are two logics you can use. Display Logic and Skip Logic. These are to display or skip a question only if a particular answer is selected, in case of multiple choice questions.
